Provides case management services to mothers and newborns experiencing medical or social risk factors that affect health and development. Case Management More information:
- Prioritizes teen mothers, individuals lacking prenatal care, substance use issues, mental health conditions, and intimate partner violence
- Serves families facing housing instability or homelessness, history of infant death or preterm birth, and very low birthweight under 1500 grams
- Addresses any additional circumstances deemed a serious risk for the mother or infant
