- Offers ramps, lifts, widened doors, railings, grab bars, stair lifts, and modified bathroom fixtures
- Provides vehicle aids such as hand controls, wheelchair lifts, spinner knobs, and pedal extenders
- Offers daily living aids, environmental controls, communication devices, hearing aids, and other equipment
- Limits funding to $7,500 per request
- Requires a 20% consumer contribution
- Provides assistance no more than once every two years
