  • Access Partnership, The

    SPECIALTY CARE: Provides low-cost or free specialty care to people who are uninsured and uninsurable.

    PROCESS:
    The patient goes to their primary care doctor at one of the locations. The doctor gives them a referral and if they are eligible will then see the TAP Navigator. The TAP Navigator will review the referral. Once the referral is approved, they will schedule an appointment for the patient and the patient will pay $20 every 90 days (which goes towards the TAP program). The TAP Navigator will call the day before and the day after the scheduled appointment to do follow-up.

    Payment Description: $20 (every 90 days).

    Intake and Eligibility: Assists children, youth, and adults are at or below 200% of the federal poverty level, live in one of the specified zip codes, uninsurable, and have a referral from their primary care doctor from one of these institutions: <br /> -Hopkins Primary Care Locations <br /> Johns Hopkins Bayview General Internal Medicine <br /> Johns Hopkins Bayview Children's Medical Practice <br /> Johns Hopkins Comprehensive Medical Practice <br /> Johns Hopkins Outpatient Center General Internal Medicine <br /> East Baltimore Medical Center <br />The primary care doctor must be from one of the locations below, no exceptions. <br /> <br />
