HOMELESS OUTREACH AND SERVICE COORDINATION: Offers assistance to eligible individuals to meet with someone who will review their situation to try and make direct connections and necessary referrals such as mental health treatment, substance abuse treatment, legal, housing, etc.

--In a shelter (although they would be better served by case management inside the shelter)

--**Individuals who spent the night in a hospital are not eligible unless the night before the hospital admission they were on the streets, in an abandoned house, in a car or a shelter.**--
--If a caller seems to qualify ask if they would be interested in meeting with someone in person who could assist them.
--Explain what HCAM Coordination will do for them.
--Explain that you will be sharing their information with HCAM and that someone will be reaching out to them in the next 3 business days. Many times it is same day, but we want to make sure to give them wiggle room.
--Explain that if they do not hear back in 3 business days to call back to 211 and ask for you. If they call back and have not heard from HCAM, tell Brandi and she will follow up.
--Read the authorization to them and ask that they agree to it.
--Fill out the form.
--Save it in the 211 shared drive in the HCAM folder and label it: Last Name and date. EX: Nieland 07.18.18
--Email the referral form to firstname.lastname@example.org and email@example.com and cc Brandi.
