    HOME HEALTH CARE: Provides skilled home care services, hospice, and response services "Lifeline" to residents of the Mid Shore region. Provides skilled nursing that assists with pain management, diabetes, wound, and catheter care. Also assists with medication monitoring, injection, intravenous therapy, and rehabilitation.

    -Home Health Aide: Assists the patient with personal in home care with bathing, toileting, dressing and ambulation.

    -Physical Therapy: Assists patients improve their ability to walk and get up and down. Therapist also determines equipment needs in the home and help reduce patients risk of falling.

    -Occupational Therapy: Helps patients to improve in daily tasks such as bathing, dressing and basic household activities.

    -Speech Therapy: Helps patients and their families with memory problems, improving the ability to communicate and restoring patients ability to swallow safely.

    -Personal Response (Lifeline): Provides immediate response to individuals in need so that they can live independently.

    Payment Description: Private insurance, Medicare, Medical Assistance, and Sliding fee scale.
