    SOCIAL WORK: Provides counseling for patients and families focused on problems relating to illness, and assistance with post-hospital planning and referral to community services. 

    FOOD PANTRY: Provides non-perishable food to individuals and families in need, 2 times a year. The food provided requires cooking, which may not be appropriate for people who do not have access to a kitchen. **Provides a grocery bag size of food. Individuals should have appropriate transportation home from the pantry. Make sure before sending the referral that the person has adequate transport home.**
